HOMEWOOD, Ill. — Uncoordinated ventilation caused a flashover that killed one firefighter and injured another when both failed to recognize signs of rapidly deteriorating conditions, investigators found.

Insufficient staffing was also cited as a key contributing factor in the incident, as crews on scene were stretched thin according to a NIOSH report released Tuesday.

Rookie Homewood Fire Department Firefighter-Paramedic Brian Carey was killed of smoke inhalation on March 30 while assisting in search and rescue of a reported victim trapped in a house fire, the report said.

Responding to reports of a downed brother, firefighters conducting a search discovered Firefighter-Paramedic Carey entangled in a hoseline and not wearing his helmet or facepiece, and without a hood.

Firefighter-Paramedic Karra Kopas, who had entered the structure along with him, was injured in the fire and had to be rescued four feet from the front door where she said her gear melted to the living room carpet.

At the time of the flashover, firefighters performing ventilation were not coordinating with hoseline and search and rescue crews inside the house, according to the investigation.

Both Firefighters Carey and Kopas were between the fire and the ventilation source.

“One firefighter accounts heavy, turbulent, black smoke pushing from a window on the B-side after it was broken,” the report said.

“Shortly after, the house sustained an apparent ventilation-induced flashover.”

NISOH says the thick, black and heavily pressurized smoke that exited through ventilation should have been acted upon as a warning sign.

“The IC, and individuals working on the exterior, need to recognize this as a potential for extreme fire behavior and evacuate interior crews,” the report said.

In addition, investigators recommend training firefighters under realistic conditions to indentify the signs of an imminent flashover.

“Obtaining proper training and hands-on experience through the use of a flashover simulator may assist interior firefighters in making sound decisions on when to evacuate a structure fire,” the report said.

The inability to appropriately coordinate fireground operations may have been directly tied to inadequate staffing.

“Due to short staffing, the ambulance personnel were tasked with fire suppression activities, thus taking them out-of-service as a medical unit,” the report said.

The incident commander, a Lieutenant, was also required to ride and operate as the officer of an Engine Crew due to short staffing.

“This removed him from his command response vehicle which would have allowed him to command at a tactical level versus having to potentially perform tasks,” the report said.

Investigators also found an accountability system was never put in place and a personnel accountability report was never conducted following the incident.

As a result of the incident, NIOSH made the following key recommendations for fire departments to follow:

  • Ensure that a complete 360-degree situational size-up is conducted on dwelling fires and others where it is physically possible and ensure that a risk-versus-gain analysis and a survivability profile for trapped occupants is conducted prior to committing to interior firefighting operations.

 

  • Ensure that interior fire suppression crews attack the fire effectively to include appropriate fire flow for the given fire load and structure, use of fire streams, appropriate hose and nozzle selection, and adequate personnel to operate the hose line.

 

  • Ensure that firefighters maintain crew integrity when operating on the fireground, especially when performing interior fire suppression activities.

 

  • Ensure that firefighters and officers have a sound understanding of fire behavior and the ability to recognize indicators of fire development and the potential for extreme fire behavior
    Ensure that incident commanders and firefighters understand the influence of ventilation on fire behavior and effectively coordinate ventilation with suppression techniques to release smoke and heat.

  • Ensure that firefighters use their self-contained breathing apparatus (SCBA) and are trained in SCBA emergency procedures.

Respiratory Diseases and the Fire Service, Report Issued

The United States Fire Administration (USFA), in partnership with the International Association of Fire Fighters (IAFF), announced the release of Respiratory Diseases and the Fire Service, a report discussing the findings of a long-term study intended to examine and improve the occupational health of firefighters. Link to Report HERE

“Respiratory diseases remain a significant health issue for firefighters and other emergency responders,” said IAFF General President Harold A. Schaitberger. “Respiratory effects due to firefighter occupational exposure are a concern affecting the fire service throughout the United States.” The goal of this project was to research the long-term effects and post-exposure mitigation of occupational respiratory exposure to firefighters and develop a report based on this research.

This effort involved renowned experts in the field of pulmonary medicine. The study will assist in recognizing and quantifying the impact of respiratory exposure, and the development of mitigation strategies and programs for firefighters, their families, and fire departments. The manual was written for fire fighters and emergency medical responders, a group of individuals who face special occupational risks of respiratory diseases due to fire ground exposures and their direct interaction and contact with the public. Respiratory diseases in fire fighters have been an area of concern and focus for the International Association of Fire Fighters and others for several decades.

Although medical progress has led to improvements in the diagnosis and treatment of respiratory diseases, prevention remains the best method of decreasing the number of such diseases and related deaths. Understanding diseases of the respiratory system, identifying respiratory disease-causing agents, and avoiding exposure to these agents are key in preventing respiratory diseases.

Excerpt from Introduction: Respiratory diseases remain a significant health issue for fire fighters and emergency responders, as well as civilians. Respiratory disease is the number three killer in North America, exceeded only by heart disease and cancer, and is responsible for one in six deaths. The American Respiratory Association estimates that more than 35 million Americans are living with chronic respiratory diseases such as asthma or chronic obstructive pulmonary diseases (COPD) including emphysema and chronic bronchitis.

Fire fighters work hard each and every day, proudly protecting and serving our citizens by answering the call for help — a call to save lives. That call may be to suppress fire and save lives jeopardized by smoke and flame. It may be a response to a hazardous materials incident, a structural collapse or other special operations event. The response may be for emergency medical assistance and transport to the hospital, with potential exposures to a host of infectious disease. Fire fighters have little idea about the identity of many of the materials they are exposed to or the health hazards of such exposures — whether they are chemical, biological or particulates.

Nevertheless, fire fighters and emergency medical responders continue to respond to the scene and work immediately to save lives and reduce property damage without regard to the potential health hazards that may exist. A fire emergency has no engineering controls or occupational safety and health standards to reduce the effect of irritating, asphyxiating or toxic gases, aerosols, chemicals or particulates.

It is an uncontrollable environment that is fought by fire fighters using heavy, bulky and often times inadequate personal protective equipment and clothing. An occupational disease takes years to develop. It is the result of a career of responding to fires and hazardous materials incidents; it is caused by breathing toxic smoke, fumes, biological agents, and particulate matter on the job; and it is the response to continuous medical runs or extricating victims at accidents. Some health effects are immediate while others may take years and even decades to develop and because some respiratory diseases develop over time, it’s impossible to say, “This specific emergency response caused my disease,” yet fire fighters continue to get sick and die from occupationally caused respiratory diseases.

Variability in exposures among fire fighters can be great; however, a number of exposures are commonly found in many fire scenarios. The common combustion products encountered by fire fighters that present respiratory disease hazards include but are not limited to: acrylonitrile, asbestos, arsenic, benzene, benzo(a)pyrene and other polycyclic hydrocarbons (PAHs), cadmium, chlorophenols, chromium, diesel fumes, carbon monoxide, dioxins, ethylene oxide, formaldehyde, orthotoluide, polychlorinated biphenyls and vinyl chloride. Also, findings from fire fighters monitored during the overhaulphase (fire is extinguished, clean-up begins and where respiratory protection is not usually available) of structural fires indicates that short-term exposure levels are exceeded for acrolein, benzene, carbon monoxide, formaldehyde, glutaraldehyde, nitrogen dioxide and sulfur dioxide as well as soots and particulates.

They are often exposed in their fire stations to significant levels of diesel particulate from the operation of the diesel fueled fire apparatus. Fire fighters are routinely exposed to respirable particulate matter consisting of liquids, hydrocarbons, soots, diesel fumes, dusts, acids from aerosols, and smoke. Health effects are known to be produced not just by the particulates themselves, but also by certain chemicals adsorbed onto the particulates. Further, the mixture of hazardous chemicals is different at every fire and the synergistic effects of these substances are largely unknown.

FDNY Citywide Tour Commander Asst Chief Gerard Barbara moments before the first collapse

For many of us, the events of September 11th, 2001 will forever be etched into our minds and hearts. The magnitude and severity of the sacrifices made that day by the FDNY as well as the NYPD, EMS and PANY/NJ uphold the tradition, beliefs, values and ideals that the Fire, Rescue, EMS and Law Enforcement professions embrace. The tragic loss of lives, the promise of the future; the unfulfilled opportunities and contributions that were yet to be recognized or made by many of those killed and the subsequent loss of completing life’s journey with their families, loved ones and comrades further magnifies the senselessness and grief many of us share to this day. FDNY Assistant Chief Gerard Barbara , the Citywide Tour Commander on the morning of September 11th (Remembrance HERE) whose image was profoundly captured standing in the street within the shadow of the twin towers moments before the first collapse provides a poignant reminder of our sworn duty, obligation and responsibilities as firefighters.

As I was preparing to capture some thoughts that reflected upon this, the ninth anniversary of 911, I came across an article that I had written within the subsequent days of September 11th that was published shortly thereafter.

As I began rereading the narrative, the vivid emotions and sentiments that were present in such a raw manner on that day and in the days and weeks that followed came rushing back to the surface. I reflected on the thought that sharing this narrative once again would echo upon some of what we all shared that day and give rise to where we’ve been in our own personal journeys. This is why we must remember, this is why we must never forget.

The First Steps of Our Journey (originally written and published September, 2001)

Tuesday September 11th began unremarkably like many others. I began my instructional delivery of a course of instruction on Incident Command Management for Structural Collapse Rescue Operations as part of the National Fire Academy’s field delivery programs in Ft. Myers, Florida. The class was comprised of Special Operations Battalion Chiefs, Command and Line Officers from throughout the region.  As we began our discussion on the needs for urban search and rescue preparedness and its relationship to strategic incident command management and tactical company level capabilities, the Ft. Myers Chief of Department came into the classroom and directed us immediately to the station day room. The time was 08:55 hours, and so began our journey.

The class immediately became transfixed upon the televised images streaming before us. The live coverage of the evolving sequence of events, the fire and emergency services responses and the devastation inflicted both in New York City and later in Washington, D.C., and the realization that this was a terrorist attack. For the next three hours we watched in disbelief the unfolding events in New York City at the World Trade Center, each of us fully realizing the magnitude and severity of the incident and the impact inflicted upon the fire, rescue, ems and law enforcement personnel operating at the scene.

The transmission of Manhattan Box 55-8087 to the World Trade Center Towers brought New York City’s Bravest and Finest. We witnessed the evolving events of the initial high-rise fires in WTC Tower #1, the vivid images of the second aircraft impacting WTC Tower #2 and shortly thereafter, the horrendous collapse of both towers.

We watched in silence, fully cognizant of the potential toll the resulting collapses could have on the operating personnel and civilians alike. Following numerous telephone calls home and to my fire station, with the impending arrangements and planning being undertaken for our fire department’s possible deployment to NYC, I began a twenty-two hour trek back home. The journey back was consumed with the constant reports filtering through the radio speakers of the ever increasing descriptions of the magnitude and levels of destruction at what has become known as Ground Zero.

The turnpikes I traveled were filled with the passing images of the initial public outpouring of emotions to the day’s tragic events. Lone individuals on overpasses and bridges, waving our nation’s flag. The flags drawn to half staff throughout the communities I passed through and the electronic message boards along the highway, with words of condolence and encouragement in this time of national grief. Still in my Fire Academy shirt with the embroidered words of the NFA and Structural Collapse, I was recognized as a firefighter and approached by numerous people along my route back who questioned the events of the day, who were seeking some sense of understanding for what was becoming recognized as a significant loss of life to unaccounted for fire, rescue, law enforcement and civilians.

There were the unsolicited words of thanks expressed by people at gas pumps and rest areas up the entire east coast, who acknowledged my fire service affiliation and connected to what they may have seen or heard in terms of the of the missing F.D.N.Y. firefighters and N.Y.P.D. law enforcement officers. This level of acknowledgement, seemed so strange, when any other time, we seem to blend into the back ground of everyday life. All for having a fire service emblem on.

During my travel back to Syracuse, New York I listened to every report, every update and the ever increasing numbers of potential missing on the radio. Well after midnight I ran into a colleague of mine at a gas station, an Assistant Fire Chief from the Metro Dade Fire & Rescue Department, Florida who, along with four other urban search and rescue specialists were making their way to Washington, D.C. as part of the deployed FEMA USAR Task Force Team from South Florida. We shared in our grief over the immediate notification at a mayoral press briefing that our close friend FDNY Battalion Chief Ray Downey was identified as one of three chief FDNY Officers who died during the tower collapses.

We also shared in our grief in the initial reports of the over forty FDNY fire, rescue and support companies unaccounted for as a result of the fire suppression, rescue and collapse efforts. The continuing ride gave way to the thoughts and concerns of many of my friends within the FDNY. Were they on shift, are they accounted for, are they safe? I thought about everything that we have tried to prepare for, the years of developing our national urban search and rescue task force system, collapse-rescue training, terrorism preparedness and the images of the WTC events of the morning. I thought deeply of my twenty-six years of fire service involvement, my brother & sister firefighters, and again- the fate of my FDNY brothers and sisters in New York City.

Subsequently in the days that followed, I became glued to the live televised images from Ground Zero and ever increasing reports of the search and rescue efforts deployed at the incident scene. As I watched alone into the early morning hours the images pouring across my television screen or at the fire station with my brother and sister firefighters, I began to contemplate the journey that lay ahead for our nation’s fire and emergency services. We will be forever changed by the events of 9-11. The most recent accounts have identified over three hundred thirty seven confirmed or unaccounted for firefighters, twenty-three law enforcement officers and over five thousand four hundred missing civilians. Rescue efforts remain the focus, with the realization that the probability of live rescues diminishes with each passing hour as the first week of Herculean efforts draws to a close.

The fabric that binds us within the fire and emergency services, the true bonds of brother and sisterhood in this proudest of professions can not be more poignantly depicted than the image of the three brother FDNY firefighters raising the American flag amidst the mountains of rubble and debris where once stood the World Trade Center. Each and every one of us understands the undertakings during the initial stages of operations at the WTC. We, the fire and emergency service providers protect the heart and soul of our respective communities. We understand the risks and challenges affecting our commitment to protect life and property and to meet those challenges armed with our training, preparedness and tools of our trade. We are the first ones in and the last ones out. The challenges ahead will be immense as the rescue efforts at Ground Zero evolve into the recovery mode of operation, and the continued efforts to bring home- back to quarters these missing firefighters.

In the days, weeks and months ahead, we will be witness to ever changing events in this continuing journey. We will share in the pain, grief and emotions that have become so deeply rooted inside of all of us in the course of these events in NYC and in our nations’ capital. For those who provided direct or support service to the events at the WTC, and those who may yet be called upon to render aide in the weeks and months ahead, each of us understands the calling and we also understand the pain. For each and everyone firefighter, rescue and ems provider would, if they could, would be side by side with those working at Ground Zero.

We must remain vigilant to our own community’s risk potential for future events and incidents and must strive to reduce the gap between our capabilities and those identified deficiencies. We must plan and train for the worst, for it’s not a matter of IF , it’s just a matter of WHEN. Our nation’s fire and emergency services have begun a journey, one that no one could have imagined, yet one that each will meet head- on. Remain safe, stay strong, and meet the challenges of your next alarm, with faith and the foundation of principles that have made our fire services what they are. We are all part of a brotherhood, we share a common belief and mission-we know our duty, we are firefighters, and will answere the call.  (September, 2001)

Honor and Remembrance

Remember and honor the sacrifices of 09.11.01 and the continuing sacrifices that are being made today by those fire, law enforcement and emergency services workers, support personnel and civilians that worked the recovery efforts at Ground Zero in the weeks and months afterwards who are dying or are afflicted by the lingering effects of exposure at the site. Remember the surviving families of those lost, remember the firefighters; who they were and remember who we are, and what we do each and every day in the streets of America. May We Never Forget. Talent is not Enough

From playing many years of sports on both winning and loosing teams I have learned talent is not enough to bring about success to the team. I played on teams that had plenty of talent. We didn’t win a lot though. That used to always bother me and I would say we have all the talent in the world why aren’t we winning. It was not until my senior year when I realized that talent was not enough. There was another component that had to be added in. That component was ATTITUDE. It wasn’t the other teams didn’t have attitude. We had plenty of it…but it was the bad kind. It wasn’t until the positive attitude mixed with talent that we started winning.

Over the years the fire service organizations that I have had the opportunity to be involved with showed me that this concept applied to any organization or team. Various attitudes have the potential to impact a team made up of talented members.
Abilities + Attitudes = Results
Good Talent + Rotten Attitudes = Really Bad Team
Good talent + Bad Attitudes = Bad Team
Good Talent + Average Attitude = Average Team
Good talent + Good Attitude = Good team
Great talent + Great Attitude = Great team
If you are looking for a winning team that performs at an outstanding level you need minimally good talent with good attitudes, preferably great attitudes and great talent!

Most people possess good talent it is usually the attitude that either draws them do to the next level with great/positive attitudes or their demise with a rotten attitude. So knowing this, it is important to choose the right attitude.

IAFC FRI Company & Command Officer Leadership Symposiums

The International Association of Fire Chiefs (IAFC) recently held its annual Fire Rescue International (FRI) Conference in the great City of Chicago. A stellar addition to extensive pre-conference and workshop presentations was the IAF’s comprehensive Flagship programs entitled; The Company Officer Leadership Symposium (COLS) and the newest addition, The Chief Officer Leadership Symposium.

New to 2010, The Chief Officer Leadership Symposium consisted of three days of education geared toward those newly promoted to battalion chief. Similar to the Company Officer Leadership Symposium (COLS), this program was built around and into a three-level course for those in various stages of their career. This year at FRI, the IAFC introduced level one for recent graduates of The Company Officer Leadership Symposium (COLS) program and those looking for education specifically geared toward battalion chiefs.

The three-day Company Officer Leadership Symposium (COLS) provided the perfect mix of what company officers are looking for and what chief’s want for their company officers. The program offerings at FRI 2010 provided in-depth leadership education like for newly promoted company officers and those transitioning to battalion chief. TheCompanyOfficer.com and CommandSafety.com’s Christopher Naum, provided a key note general session delivery at the end of day one of the three day symposium and presented a powerful and insightful look at the Doctrine of Combat Fire Engagement 2010. Presented to a joint session of students from The Company Officer Leadership Symposium (COLS), the Chief Officer Leadership Symposium and participants of the iWomen’s 2010 Leadership Conference, the multi-media lecture was presented to a standing room only crowd of over 325 participants. The Doctrine of Combat Fire Engagement 2010 examined common attributes and emerging insights related to buildings, structures and occupancies that comprise typical response districts and the unique challenges during structural fire attack that require new insights and skill sets for company and command officers and fire service personnel.

The program examined and advocated strong principled new views of various buildings and occupancies, providing examples that define and determine how firefighters access, react and expect similar structures and occupancies to perform at a given alarm. Naum introduced defining new concepts related to Tactical Patience, Command Compression, Tactical Entertainment and aligned the Anatomy of Buildings on Fire, Building Construction and Reading Building Profiles and Occupancy Risk while stressing the importance of the emerging Tactical Renaissance and continued emphasis on the Everyone Goes Home Program and 16 Fire Fighter Life Safety Initiatives. The Predictability of Building Performance and the emphasis on dynamic command risk assessment aligned with defined fire suppression operations filled the two hour session.

What a Difference… A Minute Can Make

Have you ever recognized what a difference a single minute in your life can make? Most of us only count down the minutes at the end of the day near quitting time, or when we are waiting for a big event. We never really recognize just how important every minute is because every minute makes a real difference. It is important to remember that for everything there is a season, a time for every activity.

Be Aware of Critical Moments
A critical moment is when you make a decision that has a critical impact on your life. These can include fire ground decisions, career decisions, attitude decisions or decisions on choice of words. These may last only a few minutes, hours or days. Sometimes these decisions may have impacts that last a life time. Most of our decisions are made in a rapid fire mode and are impacted by attitude. It is important to remember that attitudes are choices or decisions we make.

Some Individuals would look at a pile of rubble and say “what a mess” while others will look at the same pile and say “what an opportunity”. At this moment there is a critical decision going on. Which one of these individuals would you want leading the fire department in your community? Most would say the one who has a vision of what that “mess” could be. I recently had the opportunity to spend some time in the great State of Vermont training with a group of outstanding emergency services professionals in Addison County. What a breath of fresh air. The amount of energy that was delivered to my starving body was incredible from spending just 48 hours with such great fire service leaders. I was able to reflect upon 50+ years of leadership legacy that was still going strong. That’s right; the fire chief of Vergennes Fire Department the late Ralph Jackman had been the Chief for 50+ years. The best part was he looked at everything in a progressive, proactive philosophy of saying “look at that opportunity”. He understood that every minute made a difference and he understood these critical moments and the importance of a positive attitude even when the chips were down and things were not going as he may have hoped or wanted.

As individuals and leaders of the fire service we must look at opportunities with vision. We must be able to decode the “mess” into “opportunity”. It is paramount that we focus on the concepts that it shouldn’t be this way, but we can make it something else. These are truly hectic times we live in, times that can challenge even strongest of seasoned leaders or firefighters.

Regularly ask yourself three (3) questions…

1. Who and what is influencing me?
There are many individuals and things that can influence you. Subsequently you must ask yourself if these influences are positive or negative. Many times your influences can be strong positive ones while other times they can be the negative ones that you fall victim too. It is important to have strong positive influences in our lives. Remember ever time you choose to follow an influence it is a critical decision and becomes a critical moment in your live. “

2. Where does my mind naturally go?
What are you thinking about when you have free time or where does your mind drift off too frequently. Where your mind goes will have a big influence on critical moments in your life. Make sure that the place your mind is visiting is worth being there!

3. What am I passionate about?
What do I really like in life is another way to say this. Well often times when we get to this level of soul searching we can see that we have things a lot better than others. Often times it is a big reality check that we realize we are not following or doing our passions. It is important to make sure that your passion is not a negatively impacting one as well. Remember everything is influenced by our attitudes; you should always be reminding yourself that your attitude is like a disease and is yours truly worth catching.

Don’t Miss Opportune Moments
We should all be reminded just how brief our time being alive really is. None of us will live forever. We are merely moving shadows and all our busy rushing ends in often times nothing. Opportune moments don’t have to be big successes, but can be as simple as learning how to do something new. We are all busy and miss the opportunity to celebrate great moments. So with all this rushing around and what we are missing let’s look at what happens when you get in a real hurry or act in haste…
• You feel stressed.
• You lose your joy…simplified your laughter, special times and moments of impact.
• You are less productive.
• You can’t hear or see anyone.

So if you don’t want to miss opportune moments or act in haste you need to slow done. You may ask, “how do I slow down”? Remember it is important that you work hard but take time to rest as well. I recently was out to eat lunch. A group of pastors were at this restaurant as well. I heard one of the pastors state I always remind my congregation that the Devil never rests, another pastor asked since when did we start following his lead. Silence fell on that group for a moment and the first pastor replies wow I never looked at it like that! So what are some helpful hints to get you to slow down?
• Participate – Go and do more with family friends, colleagues.
• Delegate – Don’t put that big Superman “S” symbol on your chest. It usually doesn’t signify you are “Superman” but more like “Stupid man”
• Procrastinate – Stop and think before you act or speak, often times take more than just a second in this case. I see great leaders take days, weeks and months to act on items to keep from making poor decisions.
• Eliminate – Eliminate all the bad influences, attitudes and passions

Please don’t waste your minutes…they may be running low and you don’t even know it!

High Rise Fire Fighting Operations

Houston (Texas) firefighters followed the “textbook approach” in fighting a blaze at a high-rise building on the 27th floor Monday night August 30, 2010 when a 4th Alarm was transmitted for operations at a high rise building fire, deploying nearly 175 firefighting personnel. Seven firefighters were injured with non life-threatening. A broken pipe hampered firefighting operations leaving companies without a water source for a half-hour before they could resume structural fire fighting operations.

The fire was located at the JPMorgan Chase building (formerly the Gulf Building) at 712 Main Street, a 36-story structure, which dates to 1929 and was once the tallest in Houston. Reports indicate the building was being retrofitted with a sprinkler system that had yet to reach the upper floors. Go here for a link to the building profile.

 A Mayday call was transmitted due to a firefighter who became separated in a dark and smoky stairwell but was promptly located.
